Prólogo El Tratado de cuidados intensivos pediátricos es producto del trabajo de los 22 comités que actualmente integran la Sociedad Latinoamericana de Cuidados Intensivos Pediátricos.Bajo una guía practica, una lectura amigable y conceptos adecuados a nuestra realidad, encontraremos lo que toda persona del equipo multidisciplinario que maneja al paciente pediatrico grave en Latinoamerica debe de dominar y conocer.Nuestra ideologia aborda el desafio del cuidado intensivo pediatrico actual, que estriba en el manejo del paciente de complejidad medica que se ha desarrollado debido al avance tecnologico, abordandolo desde el punto de vista biopsicosocial y de acuerdo con los recursos disponibles en nuestros paises, integrando a la familia como base fundamental de su manejo.Con esto en mente, hace 30 años un grupo de innovadores en terapia intensiva pediatrica en Latinoamerica se dio a la tarea de conformar una asociacion en donde todos pudieran presentar y apoyarse en sus trabajos, cuya historia puede verse en los inicios de este gran libro, despues de lo cual podremos encontrar el contexto general de sus temas.El comite de RCP nos habla en forma concreta de las escalas de valoracion, los equipos de respuesta rapida (ERR), las medidas actuales de soporte vital basico y avanzado y el soporte posparo, que son la base del manejo seguro del paciente.

En esta obra se seleccionaron los temas que representan la patologia anorrectal mas comun, la que dia a dia enfrenta el coloproctologo. Los trastornos de la motilidad gastrointestinal han cobrado gran importancia debido a un mayor conocimiento de los mismos y a las nuevas clasificaciones que estableció la reunión de consenso de Roma llevada a cabo en 1999, conocida como Criterios de Roma. Los Criterios de Roma III se establecieron en el año 2006.Los trastornos motores de dicho grupo afectan la calidad de vida de los pacientes, pueden dañar cualquier segmento del aparato digestivo y se caracterizan por ser funcionales y de dificil manejo, lo cual origina situaciones complicadas en la relacion medicopaciente. El concepto de tubo digestivo ha sido ampliamente superado a la luz del conocimiento de un "cerebro intestinal". Es un complejo sistema neuroendocrino que posee neuronas aferentes y eferentes, neurotransmisores presinapticos y postsinapticos, y excitadores e inhibidores de accion rapida o lenta que condicionan estas respuestas. El mediador bioquimico mas importante implicado en la fisiopatologia de la dismotilidad es la serotonina, que se localiza en 95% en el aparato digestivo. Tambien es muy importante la accion colinergica en la fisiopatologia, porque favorece la irritabilidad del mecanismo neuromuscular del intestino.El impacto en la calidad de vida y su alta prevalencia en poblaciones economicamente activas con una repercusion economica importante le han conferido un interes particular a este grupo de padecimientos. Varios estudios han documentado el impacto del sindrome de intestino irritable (SII), por ejemplo, en la calidad de vida y el desempeño laboral. En Mexico los doctores Schmulson y Valdovinos han informado en la Revista de Gastroenterologia de Mexico acerca de esta relacion, con una disminucion en la calidad de vida y en el rendimiento laboral de los pacientes. Los estudios de Hungin y col. han documentado tambien la repercusion economica de estas patologias, situacion que ha sido estudiada mediante el ausentismo laboral en Brasil y EUA con la observacion de que los trastornos motores producen un mayor ausentismo laboral al compararlo con los grupos controles (de 1.5 a 5% vs. de 4 a 10% en SII). En este estudio los autores informan que 36% de los pacientes con SII de EUA y de 10 a 29% de los pacientes de los paises europeos han perdido su trabajo en el ultimo año por ausentismos debidos a dichos sintomas. Es muy conveniente recordar que cerca de un tercio de las consultas del especialista se deben a trastornos funcionales digestivos y que el numero de visitas promedio por paciente es de dos a tres por año.

La Asociación Mexicana de Gastroenterología ha tenido un gran acierto al incluir en las Clínicas de Gastroenterología de México esta publicación, que enfatiza los conceptos del trabajo en equipo. Hemos seleccionado cuatro temas en donde esta colaboracion es mas patente, participando endoscopia, radiologia y cirugia en cada uno de ellos. Los dos primeros tratan sobre la hemorragia digestiva por hipertension portal y la no relacionada con hepatopatias. Estos temas han sido motivo de preocupacion y de discusion por muchos años; ya nadie puede discutir que la endoscopia es la primera opcion de tratamiento para la hemorragia y que la radiologia puede resolver un buen numero de casos en los que la endoscopia no lo consigue, y que finalmente, en un grupo reducido de pacientes, la cirugia es la solucion final en estas entidades dramaticas y que a pesar de todo siguen siendo causa de mortalidad en todos los hospitales del mundo, sin importar sus recursos o su experiencia.Los otros dos temas, divididos en seis capitulos, se refieren a las enfermedades del arbol biliar, la coledocolitiasis y la lesion iatrogenica de la via biliar, que son problemas muy importantes en donde nuevamente se debe tomar la decision de manera colegiada, proporcionando al pacientela mejor alternativa e incluyendo los recursos de cada institucion, la experiencia de cada grupo de trabajo y, sobre todo, pensando en el bien del paciente.
